Most of us measure age by the calendar. Biological age is different: it is an estimate based on selected biological measurements that are compared with patterns observed in populations of different ages.
The idea is scientifically interesting, but biological age testing is still developing. These tests are not established diagnostic tests, and a biological age result cannot diagnose a disease, rule one out, or replace medical assessment or recommended screening.
That distinction matters. A biological age number can provide context around certain health measurements, but it should not be treated as a verdict on your health or as a prediction of exactly how long you will live.
What Is a Biological Age Test?
A biological age test uses a set of measurements to estimate how closely your results resemble patterns seen in people of different chronological ages.
Depending on the method, testing may use routine blood markers, metabolic measures or patterns of DNA methylation. These measurements are combined into a composite score or estimate.
Biological age is therefore not one universally defined measurement. Different tests use different markers and algorithms, which means two tests can produce different estimates for the same person.
Biological Age Versus Chronological Age
Your chronological age is simply the number of years since you were born. A biological age estimate attempts to describe aspects of biological variation associated with ageing.
Health-related measurements can change over time. Blood glucose, blood pressure, lipids and other markers may respond to changes in physical activity, diet, sleep, alcohol intake, illness and other factors.
However, a change in a biological age score should be interpreted carefully. If a result changes when you test again, that difference may reflect a genuine change in the underlying measurements, normal biological variation, technical variation or differences in the testing method.
How Is Biological Age Calculated?
There is no single biological-age test. Several approaches have been developed, and they measure different aspects of ageing.
Blood-based biological age measures
Some biological-age algorithms use routine blood measurements such as glucose, albumin, blood-cell counts and other clinical markers. These approaches can combine information that is already familiar in conventional health assessment.
The advantage is that the underlying measurements can often be interpreted in their usual clinical context. The limitation is that combining them into a biological-age score does not automatically make the score a diagnostic measure.
DNA methylation testing
DNA methylation describes chemical changes associated with DNA that can vary with age and other biological influences. Researchers have developed several “epigenetic clocks” that use DNA-methylation patterns to estimate biological age.
These methods are an active area of ageing research. Different clocks can provide different estimates, and research has identified both biological and technical sources of variability. For that reason, an epigenetic age result should be interpreted as a research-derived estimate rather than as a definitive clinical measurement.
What Can a Biological Age Test Actually Tell You?
The practical value of a biological age assessment usually comes from the measurements behind the final number.
Depending on the test, a report may include:
- Metabolic measures such as glucose and lipids
- Selected inflammatory markers
- Blood-cell measurements
- Other cardiovascular or metabolic indicators
- DNA-methylation measures in some testing methods
A result may prompt useful questions about lifestyle or whether a conventional health assessment is appropriate. But an apparently favourable biological age does not prove that you are healthy, and an unfavourable result does not by itself establish that you have a disease.
What These Tests Cannot Replace
A biological age result should not be used as a substitute for clinical assessment or Australia’s recommended screening and preventive-health programmes.
For example, a biological age result does not replace appropriate blood-pressure assessment, bowel screening, breast screening, cervical screening or assessment of symptoms.
If you have a health concern or symptoms, the appropriate starting point is a qualified health professional rather than a biological-age score.
Why Repeatability Matters
One of the important research questions is how consistently biological-age tests produce results when testing is repeated.
Recent research has highlighted both biological and technical variation in epigenetic clocks. Different methods can respond differently to the same sample, and a small change in a score may not represent a meaningful change in health.
This is why it is better to focus on trends in relevant underlying health measures alongside the context of the test, rather than treating a one- or two-year change in biological age as proof of accelerated or reversed ageing.
Availability in Australia
Blood-based and DNA-methylation testing are available through a range of providers in Australia. Some tests are arranged through practitioners, while others are offered directly to consumers.
Before paying for a test, ask which laboratory processes the sample, what method is being used, what measurements are included and how the result will be interpreted.
It is also useful to ask whether a qualified practitioner will discuss the results with you, and what the test can and cannot tell you. Be cautious of services that present a biological-age score as if it were a definitive health diagnosis or a precise measure of future lifespan.
Understanding Your Results
A report that gives you one number without explaining the underlying measurements is of limited use.
Interpretation should consider your medical history, medicines, current symptoms and the circumstances surrounding the test. Recent illness, changes in sleep, fasting status and other factors can influence individual biological measurements.
The most useful outcome may therefore be identifying a conventional health measure that deserves attention, rather than trying to make a larger conclusion from the biological-age number itself.
Can You Influence Biological-Age-Related Markers?
The factors that influence many of the measurements used in biological-age research are also factors that influence general health. Importantly, you do not need a biological-age test before making healthy lifestyle changes.
Food
A balanced eating pattern based on a variety of nutritious foods, including vegetables, fruit, wholegrains, sources of protein and healthy unsaturated fats, supports overall health. The Australian Dietary Guidelines provide general advice for healthy eating.
Physical activity
Regular physical activity supports cardiovascular fitness, metabolic health, muscle strength and overall function. Australian guidelines recommend adults aged 18–64 accumulate 2.5 to 5 hours of moderate-intensity physical activity each week, or 1.25 to 2.5 hours of vigorous activity, together with muscle-strengthening activity on at least two days each week.

Sleep
Regular, adequate sleep supports general health. Persistent loud snoring, waking unrefreshed or excessive daytime sleepiness may warrant discussion with a doctor because sleep disorders can require assessment and treatment.
Stress
Long-term stress can affect sleep, blood pressure, activity and eating patterns. Helpful approaches vary between people and may include regular exercise, structured relaxation, social support or psychological care where appropriate.
Alcohol and smoking
Reducing alcohol intake and avoiding smoking are established ways to support long-term health. Australian guidelines advise healthy adults to drink no more than 10 standard drinks per week and no more than 4 standard drinks on any one day.

Biological Age Testing Alongside Other Assessments
Biological-age testing should be considered alongside, rather than instead of, conventional health assessment.
Depending on your circumstances, a clinician may consider established assessments such as cardiovascular risk assessment, routine blood tests or other investigations. Tests such as DEXA scans or hormone assessments have their own specific purposes and should be considered based on clinical need rather than as an automatic checklist.

Biological age testing is an interesting research-derived approach to understanding patterns associated with ageing. It can provide a different way of looking at a collection of biological measurements, but the headline number has important limitations.
